If you have insurance then call your insurance plan first.

The terminology for addiction has changed over the years. Under ICD-10 and DSM V, it is currently called Substance Use Disorder. Most people still understand what is meant by addiction, so when you are searching for treatment help and information do include the term “addiction” in your searches.

Ask if opioid addiction/substance use disorder treatment is covered under your plan, if yes, … then ask for a list of contracted providers, or specifically a list of contracted Suboxone providers. Not all addiction treatment providers are Suboxone prescribers.

State of Tennessee calls it Substance Abuse Services. You can find the services provided by the state from their website https://tn.gov/behavioral-health/section/substance-abuse-services or by calling their help line at 800-560-5767.

I do not accept any insurance for Suboxone/Subutex therapy.
There is no point in calling doctor’s offices and asking them if they accept insurance.  Instead … call your insurance plan and get the names and phone numbers of doctors who are in your network that provide office based Suboxone/Subutex therapy.

Tenncare provides prescription coverage for Suboxone for a limited period. The prescription plan is managed by Magellan Health Services.

Tenncare Preauthorization Form

Some of its policies are:

• Preauthorization is required.  (The preauthorization form does not spell out the criteria for coverage, or the conditions attached for continued coverage. This trick is primarily used to deny coverage).
• Suboxone is only approved for three months, maximum of three tablets a day, to be used once daily.  After three months preauthorization has to be filed again.
• Patient has to be enrolled with an addiction counselor, and that information has to be provided on the pre-authorization form.
• If you were getting Suboxone through Tenncare plan and you changed provider then this preauthorization has to be filed again, and your previous provider has to transfer the care.
• Preauthorization requires certification by provider, and asks for information that the provider does not have, e.g, about your prior Suboxone therapy, counselor etc.
• It is best to go through a treatment facility if you have Tenncare as they are more familiar with the rules. (Once you have gone through a facility you do not get Suboxone again)

Take your prescription plan and plan ID number with you. Have your counselor fax a note to your physician which includes the following information (you may be required to sign a special release of information form):

Patient name and current address, date of birth, insurance carrier, and insurance ID number.
